假條的格式由標題、稱呼、正文、落款等部分組成，具體寫作方法如下：
標題：“請假條”三字寫在第一行中間位置。
稱呼：在第二行頂格寫明向誰請假。寫給收條人的稱呼同一般書信。一般都是向頂頭上司或老師請假。具體寫法是在姓後面加上職務，如“陳主任”，“王科長”，“張老師”等，後用冒號。
正文：寫清請假的緣由，請假的起止時間。在第三行空兩格寫起。最後常以“請予准假”，“此致敬禮”，“不勝感激”等用語結束。
落款：署名和日期的寫法同一般書信。在右下方署上請假人姓名。姓名下方標註請假日期。
要寫好請假條.必須注意以下幾點：①請假原因要具體清楚，不能含糊其辭，模稜兩可。②如果遇到突發事情來不及請假，可請人將請假條捎給收條人。如果是事先知道的事情應提前向有關人員請假，得到批准方可。一般請假人不應起級請假。③請假時間一定要寫清楚，便於有關人員考慮和安排。④請假人在假期滿後應主動銷假，假條不必收回。
